Photos: size 5x5 cm, white background, taken within the last 6 months.
Vaccination records: certificates of previously received vaccines.
Personal medical documents: medical records, prescriptions (if any), or health history files.
Compliance with the medical examination process for us immigration

Applicants receive a “tracking sheet” confirming the completion date, a cd with the chest x-ray, vaccination records, and qft test results (if applicable). Choose the right medical facility: Only designated facilities are accepted. Results from other hospitals will be rejected.
Pregnant women: Must inform the facility in advance to apply protective measures during X-rays or to postpone certain tests.
